Below you’ll find the enrollment form for our preschool classes. For your convenience, you have two options:

2. Fill out the form directly in the window below, then download or print and sign when finished.

👉 Please note: There are two additional required forms—a permission slip and a medical statement. Links to these forms are included at the end of the enrollment form.
