Below you’ll find the enrollment form for our elementary classes. Click the Print Option to print and fill out OR you can fill the enrollment form out online- just click the button below.
👉Please note: There are three additional required forms—a permission slip, a child enrollment and health information form, and a medical statement. Links to these forms are included at the end of the enrollment form and at the bottom of this page.
Once all forms are completed, you can email them to:
